Annual wellness visits, Medicare coordination, and longer appointments built for the conversations that matter — about medications, mobility, memory, and what comes next.
What’s included
Most of our senior patients are on Medicare or Medicare Advantage. We accept both, we know the paperwork, and we won’t waste your visit talking about it.
For caregivers
Adult children, spouses, and caregivers are part of how senior care actually works. We encourage you to come along, take notes, and ask questions — with your loved one’s permission, of course.
We’ll send visit summaries home, walk through medication changes carefully, and pick up the phone when you call back later with the question you forgot to ask.
